Steps in getting an RFID/Access Badge
- Secure all the necessary requirements. Please refer to the table below.

- ID form from HRMO
- Proof of payment

- Study Load
- Lost or Stolen RFID of Students, Faculty and Personnel - Affidavit of loss
- For All Destroyed, Surrendered, Expired, Change of Course and Tampered RFID/Access Badge -ID replacement form
- Proceed to the ID Room. TC ID Room is located at the basement of the Bunzel building.
- Present your requirement(s) to the ID Personnel.
- Have your picture and signature taken.
- Check that all the information to be printed is correct prior to the printing of the ID.
- Sign the logbook upon the release of your RFID
- Verify that your RFID/Access badge is functional before leaving the ID Room. Any loss or damage done to your RFID/Access badge after leaving the ID Room will not be replaced for a free.
